Plano Family Clinic's registration process may be completed by printing the forms below. You can then fax or bring them to our office at the time of your visit. This will allow us to verify your insurance in a timely manner.
Please be advised that our office will also need a copy of your insurance card, both front and back. If you fax these forms, please include a copy of your card at that time. If you bring the completed registration forms with you, please arrive 20 minutes prior to your visit for our insurance department to verify your benefits.
PATIENT PORTAL
Online Access through the Patient Portal
If you are an existing patient, we have a free service available called the "Patient Portal." The link below allows you to communicate in a secure manner directly with members of our health care team.
To use the Patient Portal, you must obtain a username and password. You must have a permanent email address that you check consistently. Please contact our office and we will assign you a username and a temporary password.
Important Information regarding the Patient Portal:
Use is limited to non-emergency communication and requests. The Portal facilitates communication between appointments. However, the Portal does not replace your scheduled clinic appointments. It is not checked on the weekends. Please allow up to 72 hours to respond to communications and requests. We will not send any private health information to your e-mail (non-secure). We will send you an e-mail only when necessary, to request that you access the secure Patient Portal to review private healthcare information that we have posted on your Patient Portal. If you have a true medical emergency, please go to the nearest ER or call 911.
~ Hanh-Dieu Nguyen, MD ~
Dr. Hanh-Dieu Nguyen has been in practice for over twenty years. Dr. 'N' received her medical degree from University of Florida College of Medicine. She then completed her residency at Tallahassee Memorial Hospital.
Dr. N and her team aim to provide a comfortable atmosphere for patients at Plano Family Clinic. She enjoys Family Medicine because of its emphasis on prevention of disease. Her approach to medicine is to engage patients in their healthcare and to recognize the sociocultural dynamics that may impact an individual's illness.
Our Physicians
~ Khanh Truong, DO ~
Dr. Khanh Truong, originally from Danang, Vietnam, moved to Dallas at the age of 15. He graduated with majors in Chemistry, Biology, and Mathematics from Texas A&M University - Commerce and later earned a Master's degree in Chemistry. He then attended Touro College of Osteopathic Medicine in New York and completed his residency in Family Medicine at UT Southwestern Medical Center. Currently, he is pursuing a Sleep Medicine fellowship at the same institution. Dr. Truong has received recognition for his work, including being featured in TAFP and serving as Chief Resident at Family Medicine UT Southwestern. He is a member of the Texas Academy of Family Physicians and the American Academy of Sleep Medicine. Dr. Truong is board-certified in Family Medicine and specializes in knee and shoulder joint injections. Outside of work, he enjoys exploring new restaurants as a food enthusiast.
~ Gina Do, MD ~
Dr. Gina Do is a DFW native and board certified in Family Medicine. She majored in Biology at Texas A&M and received her medical degree at Texas A&M Health Science Center. She completed her family medicine training at UT Southwestern and has been in Dallas since. In her free time, she enjoys playing board games, trying out new restaurants, and spending time with friends and family.
